Sage Urgent Care is in network with Horizon Blue Cross Blue Shield of New Jersey and with Horizon NJ Health. If you carry a Horizon card, you can walk in to our Berkeley Heights clinic any day from 8 AM to 8 PM and pay your plan’s urgent care cost-share — typically far less than an emergency room visit.

What you’ll pay
Your cost depends on your plan: most Horizon members pay a set urgent care copay, while high-deductible plans apply the visit to your deductible first. Either way it is usually a fraction of what the same problem costs in an emergency room. X-ray and lab work done during your visit are billed to Horizon under the same claim. Horizon NJ Health members generally pay nothing for covered urgent care.
We verify your Horizon eligibility electronically when you check in, so you know you’re covered before you see a provider.
What you can walk in for
- Illness: strep throat, flu, COVID, sinus and ear infections, UTIs, pink eye and rashes
- Injury: sprains, suspected fractures with on-site X-ray, cuts that need stitches and minor burns
- Testing: rapid strep (5–10 minutes), flu and COVID (about 15 minutes), urinalysis, EKG and lab work
- Prevention and forms: flu shots and Tdap, sports and school physicals, and work or school notes

- Your insurance card (front and back) and a photo ID
- A list of your medications and allergies
- Any school, sports or work form you need completed

When an X-ray is medically needed during your visit, it is billed to Horizon with the visit. Your share depends on your plan’s deductible and coinsurance.
