Physician-owned, physician-led care

Sage is owned and run by two board-certified emergency medicine physicians who practice here and set the clinical standards: Dr. Jacob Silberstein and Dr. Vadim Fradlis.

We are independent and single-location — not a franchise.

From the Rose City to Springfield Avenue

From downtown Madison, head south through Chatham Township toward Berkeley Heights. The drive is short, and there is free parking in our own lot right at the door, so there’s no hunting for a spot on Main Street.

Common reasons Madison patients walk in:

For Drew and FDU students

If you’re away from your home doctor, Sage is a straightforward place to be seen: walk in, or reserve a spot online so you can wait in your dorm instead of our waiting room. We can test for strep, flu and COVID on site, treat UTIs and sinus infections, and write a note for class or practice. Bring your insurance card, including a student health plan card if you have one. Self-pay rates are available if you’d rather not use insurance.

Madison High School athletes

For Madison High School athletes, we handle in-season injuries and pre-season physicals. We can X-ray a possible fracture, splint it and give return-to-play guidance in one visit, and refer to orthopedics when needed. The NJ pre-participation physical is $75 self-pay, walk-in.

What a Madison visit costs

Families and students can use most major plans at Sage (Aetna, Horizon BCBS NJ, Cigna, UnitedHealthcare, Medicare and more, listed on our insurance page). If you’d rather pay directly, most visits are $150 and athletic physicals are $75.

What brings Madison patients in

With two university campuses and a full school sports calendar in a borough of about 17,000, Madison visits usually look like this:

  • Students without a local doctor: same-day evaluation for sore throats, flu, UTIs and sinus infections, with rapid testing on site.
  • Weekend sports injuries: sprains and possible fractures answered in one visit with on-site X-ray.
  • Kids’ illnesses after hours: fevers, ear infections and coughs when the pediatrician’s office is closed.
  • Commuters on the Morristown Line: open until 8 PM, so you can be seen after getting home from the city.

Is it an urgent care visit?

Sore throat, UTI symptoms, a sprain, a cut that won’t stop oozing: these are urgent care visits, and Sage can usually diagnose and treat them in a single trip with testing and imaging on site.

For a true emergency, such as chest pain, a head injury with confusion or trouble breathing, call 911 or go to an emergency department.
