The pre-participation exam is designed to identify any condition that might affect athletic safety:
- Detailed medical and family history — including any history of fainting, chest pain during exercise, or family heart conditions (especially under age 50)
- Cardiovascular evaluation — heart sounds, blood pressure, pulse
- Musculoskeletal exam — joint stability, prior injury assessment, flexibility, posture
- Vision and hearing screening
- Lung function — especially relevant for asthmatic athletes
- Concussion history review and return-to-play assessment if applicable
- Form completion and signature — NJSIAA, school district, camp, or college forms

A sports physical (also called a pre-participation exam) focuses specifically on whether it's safe for an athlete to participate in their sport. It covers cardiovascular health, musculoskeletal evaluation, and screening for conditions that could be aggravated by exertion. An annual physical is more comprehensive and covers preventive care, vaccinations, and overall wellness. Some kids need both — your school will tell you what's required.
Most kids with managed conditions (asthma, controlled epilepsy, ADHD, prior concussions, glasses, hearing aids) are cleared to play. Bring documentation of any condition and current medications. If we have concerns, we'll discuss them with you and your child's primary care doctor before signing off.
Bring (1) any forms your school or league requires, (2) a list of current medications and allergies, (3) records of any chronic conditions or recent injuries, (4) glasses or contacts if they wear them, and (5) their inhaler if they have asthma. A parent or guardian must accompany any child under 18.
Yes. We complete NCAA, club sports, intramural, and college pre-participation forms. Many colleges have specific paperwork — bring whatever your school sent, and we'll work through it together.
