Strep throat is one of the most common infections we test for at Sage Urgent Care. It's caused by Group A Streptococcus bacteria, and unlike most sore throats — which are viral and resolve on their own — strep needs antibiotics. Untreated strep can lead to complications including kidney inflammation, rheumatic fever, and the spread of infection to people around you.
The good news: strep is easy to test for and easy to treat. A rapid strep test takes about 5 minutes from swab to result. If it's positive, we prescribe antibiotics on the spot.
When to come in
You should get tested if you have a sore throat plus any of these:
- Fever (especially 101°F or higher)
- White patches or pus on the tonsils
- Tender, swollen lymph nodes in the neck
- Headache, stomachache, or nausea (more common in kids)
- Sudden onset (strep usually comes on fast)
- A child or family member recently diagnosed with strep
- A red rash with a sandpapery feel (scarlet fever — a strep complication)
Cough, runny nose, and hoarse voice are usually signs your sore throat is viral, not strep — but the only way to know for sure is to test.
What happens at your visit
- Quick check-in at the front desk.
- Provider exam. A physician or NP looks at your throat, checks your lymph nodes, and takes your temperature.
- Throat swab — quick and uncomfortable for a few seconds, but not painful.
- Rapid test result in about 5 minutes. If positive, we move to treatment immediately.
- Optional throat culture. If the rapid test is negative but the provider still suspects strep, we can send a culture to confirm (results in 24–48 hours).
- Antibiotic prescription sent electronically to your pharmacy of choice if positive.
Most strep visits at Sage take 30–45 minutes total.
How we treat strep
Standard treatment is a 10-day course of penicillin or amoxicillin. For penicillin-allergic patients, we prescribe an alternative like azithromycin or clindamycin. You'll typically feel better within 24–48 hours of starting antibiotics, but it's important to finish the full course.
You're considered no longer contagious about 24 hours after starting antibiotics, which is when most schools and workplaces will allow a return.

Rapid antigen tests for strep are about 95% accurate when positive. False negatives can happen, especially early in the illness, which is why we sometimes send a backup throat culture.
We don't prescribe antibiotics for sore throats without confirming strep. About 70% of sore throats are viral, and antibiotics don't help — they only contribute to antibiotic resistance and can cause side effects.
Strep is contagious until you've been on antibiotics for at least 24 hours. Most schools and workplaces require this before returning.
Recurrent strep (multiple infections in a year) is something we'd want to discuss in detail and possibly refer to ENT for evaluation. We can coordinate that referral during your visit.

A negative rapid strep test rules out strep with about 95–99% accuracy. In children with strong clinical suspicion — fever, no cough, swollen lymph nodes, pus on the tonsils — we may send a throat culture as a backup (results in 24–48 hours). In adults, a negative rapid test is generally sufficient to rule out strep without a backup culture. If strep is ruled out, we evaluate for other causes of your sore throat the same visit.
Yes. Strep spreads through respiratory droplets and direct contact — adults with strep can transmit it to household members, including children, before they realize they're infected. If one household member is diagnosed with strep, others with sore throat symptoms should be evaluated. Asymptomatic household members do not routinely need testing unless there is a documented outbreak or high-risk situation.
Most cases resolve on their own, but untreated strep can lead to rheumatic fever, kidney inflammation, and abscesses around the tonsils. It's also highly contagious. Treatment is simple and prevents these complications.
